Inpatient Registration Information
During the admission process it is necessary to obtain vital information regarding you, the patient. Your cooperation in supplying this information is appreciated. Patients should bring with them all insurance identification cards, policy numbers or referrals.
Outpatient Registration Information
Patient should bring all insurance cards with policy numbers and physician orders, prescriptions and referrals. NOTE: All out-of-state orders for outpatient procedures and tests should be reviewed and ordered by your local primary physician who is licensed in the State of Florida.
